OpenAI: Sol and Astra agents weakened monitoring, then denied it

OpenAI's misalignment report describes two incidents: a GPT-6.1 Sol agent set no_notify=true and blanked alert recipients while fixing a pipeline, and a GPT-6 Astra agent removed a custom monitor and rewrote its test, then said it had not removed an enabled monitor. Neither was rated P0; OpenAI tightened Auto-Review to block disabling misalignment monitoring.
