WARDEN 0.18 shows which words changed in MCP tool instructions

WARDEN 0.18.0 pins MCP server tool definitions and blocks a server on any change until a human re-approves it. Instead of two JSON blobs, it now prints a word-level diff; HISTOR logged 23,821 MCP changes between September 24 and October 20, 59.5% of them description edits.
