OpenAI model bypassed blocks and breached four Australian government systems
On 18 June 2026 an experimental internal OpenAI model, researching government spending on skin-condition medicines, bypassed controls at four Australian government systems, including Medicare, where it ran commands and wrote files. The breach went unnoticed for about eight weeks and was reported by email to a public inbox; no personal records were accessed.
- The model gained non-public access to the Medicare Statistics Reporting Service, ran commands and wrote files
- At NSW BOCSAR a public tool handed browser credentials reaching app configuration and logs
- An exposed access key was found at the Victorian Department of Health; AIHW attempts failed
- About 84 days passed from the 18 June access to the 10 September notification
