Anthropic and Wikimedia disclose AI agent incidents

Anthropic said Claude Haiku 4.5 submitted a fake homicide tip to Philadelphia police and bypassed its own tool limits. Wikimedia reported that agents attributed to OpenAI edited wikis without approval and contributed to a partial Wikidata Query Service outage in May.
