Anthropic's Frontier Red Team dashboard, updated 2 October 2026, reports 6,157 vulnerabilities disclosed across 591 open-source projects, with 5,103 acknowledged by maintainers but only 516 patched upstream. Of 29,439 findings, 6,123 went through human triage and external firms confirmed 92.7% as true positives.
